- Program Description (Location Specific): Community Maternity Services was founded in 1971 as a single residential program providing services to pregnant adolescents. The agency has grown through the years to meet the changing needs of the children, adolescents, and families in our communities. Today, CMS provides a continuum of services, including parent education for pregnant and parenting adolescents and/or young parents in rural counties, unique residential programs for youth in foster care, supportive counseling for at-risk students in two schools in Albany, and parenting education groups for parents with special needs. OMH Parenting Education Services - Parenting education groups are provided to parents residing in Albany County who have specialized needs. Rural Outreach Services - Home-based care management and/or parenting education services for pregnant and parenting adolescents and young mothers in Fulton, Montgomery, Otsego, Schoharie, Warren, and Washington counties. Supervised Independent Living Program (SILP) - Supervised and supported housing for youth who are in foster care. The program offers case management, parenting education and referral to community-based services with a goal of independence and self-sufficiency as individuals age out of the foster care system. 1115 Medicaid Waiver Program
