Diabetes Prevention Program

Diabetes is one of the biggest health challenges affecting adults in the United States. About 37 million people have diabetes. About 96 million adults have prediabetes, which puts them at high risk of developing type 2 diabetes. That’s more than 1 in every 3 adults in the United States.

Type 2 diabetes is a serious condition that can lead to health issues such as heart attack, stroke, blindness, kidney failure, and lower limb amputation. Having prediabetes means your blood glucose level is higher than normal but not high enough to be diagnosed with diabetes. Without making healthy lifestyle changes, such as losing weight, increasing physical activity, and eating healthier, many people with prediabetes will develop type 2 diabetes within five years.

However, prediabetes is reversible; we can help you make those healthy lifestyle changes.

The Care Coordination DPP is a year-long program that meets once a week for sixteen weeks, then once every other week for two months, and then once a month for the last six months—a worthwhile commitment to lower your risk of type 2 diabetes by as much as 58%.
